Please provide the items below as they become available, depending on the stage of your dwelling. You do not need to wait until all documents are available to submit this form.

Email documents to info@yourhomesda.com.au, including the dwelling address in the subject line. You can attach the files or provide a shared folder link, such as Google Drive or OneDrive. Please ensure we have access to view and download the files.

Owner, property and design documents

• Council rates notice

• ASIC Company Extract/Statement, if the owner is a company

• Trust Deed, if the owner is a trust

• Full building plans and specifications

• Site plans, for multi-dwelling properties

• SDA Design Certificate, Design Summary Assessment and Design Checklist

• Confirmation of SDA design notification to the NDIA, usually provided by the SDA Assessor

Marketing materials

• Marketing photographs or renders, with at least five images for Housing Hub

• Floor plan, including confirmation that it can be used for marketing

On completion of the property

• Dwelling Certificate of Occupancy

• Dwelling Statement of Compliance (Builder)

• Form 2 ESP Compliance Certificate

• As-built / final plans

• SDA Final-As-Built Certificate, Final-As-Built Summary Assessment and Final-As-Built Checklist

• Form 1 & 2 - Essential Safety Provisions (Pleas ensure this includes Fire Sprinklers, if applicable)

• Evidence of fire-resistant walls, where multiple dwellings share a wall

• Photographs showing the front of all dwellings, driveways and front and rear yard fencing, for multi-dwelling properties

• Photographs of ensuite door locks, where participant bedrooms share a bathroom

• Property Insurance Certificate

We may request updated documentation at the time of enrolment, depending on the age of the evidence being provided to the NDIA.
